Reportable Incident Report

This form must be completed by staff who witnessed a reportable (serious) incident in an assisted living environment.

A Reportable Incident must be reported to the Assisted Living Registry within 24 hours of the incident. Please review Section 51 and Schedule E of the Assisted Living Regulation. This form can be completed and signed by whichever staff member witnessed or was involved in the follow-up to the incident. Complete one form per incident.
