For submitting Notice of Assessment documentation to the Ministry of Health's publicly funded In-Vitro Fertilization Program.

Please use this form to securely send Notice of Assessment (NOA) documents to the Ministry of Health for the Publicly Funded In-Vitro Fertilization Program income verification. Ensure that you include appropriate and accurate information.

This form is for submitting Notice of Assessment (NOA) documents only, please do not submit questions or inquiries. More information on the publicly funded IVF program is available on the Government of BC Website: Publicly Funded In-Vitro Fertilization (IVF) Program - Province of British Columbia.

Ensure NOA submitted is for the current tax year
Ensure ALL pages of NOA are included in submission
Ensure your name and address are included in the NOA document
Documents other than NOA or notice of reassessment (NOR) will not be accepted
Do not submit your income documents more than once
Spelling errors will not impact your submission
