For the purpose of delivering and confirming that you have received your e-Gift card, your personal information is collected through Virtual Incentives under section 26(c) and section 26 (e) of the Freedom of Information and Protection of Privacy Act. Please be aware that your IP address, email address and e-Gift details will be collected by Virtual Incentives. This information will be stored and may be accessed outside of Canada.  

 

If you do not consent to register your e-Gift card through Virtual Incentives, the project team can offer you a physical card.

COVID-19 Content Testing – 23-003 (the "Project").

The Province of British Columbia (the "Province"), Ministry of Citizens’ Services (the

“Ministry”) is requesting that you sign this Confidentiality Acknowledgement as a

requirement of your participation with the Project.

Kindly confirm your acknowledgement as follows:

1. You acknowledge that the information that will be disclosed to you regarding the

Project, or provided to you in subsequent communications whether in written or oral

form, (collectively the "Information") is, by necessity, sensitive and confidential to the

Province.

2. You acknowledge that some of the Information is proprietary and confidential and that

disclosure would be detrimental to the Province and contrary to the public interest.

3. You undertake not to disclose the Information, except:

a. To another member of the Project Team;

b. To other employees of the Province or Contractors to the Province who agree to

be bound under the same terms as provided by this Confidentiality

Acknowledgement; or

c. In order to fulfill your obligations to the Project as directed by the Ministry as

described in paragraph 4.

4. You undertake to keep all Information strictly confidential and without the express

written consent of the Ministry first being obtained, not to disclose any portion of the

Information to any other person, organization, corporation or other entity, nor will you

permit or suffer any of the Information to be used, divulged, given or released, to any

other person, organization, corporation or other entity.

5. If any portion of this Confidentiality Acknowledgement is found to be invalid or

unenforceable by law by a Court of competent jurisdiction then that portion shall be

severed and the remaining portion shall remain in full force and effect.

6. The terms of this Confidentiality Acknowledgement are in addition to any oath of

confidentiality taken by you as a provincial employee, or any confidentiality

acknowledgement requirement imposed by statute or by an existing nondisclosure

agreement.

 

Exceptions to your confidentiality obligations are as follows:

a. If disclosure is required by a court of law;

b. If Information which has been received by you has been received from another,

non confidential source; or

c. Information which has been previously confidential is subsequently publicly

disclosed by another means.

If you confirm and acknowledge the terms as set out above, kindly so indicate by signing a

copy of this Confidentiality Acknowledgement and returning it by hand, mail, fax or courier
