About this form

This form is intended for mangers, supervisors and/or ethic advisors within the BC Public Service.

Use this form when a conflict of interest disclosure needs a formal resolution. As outlined in the Conflict of Interest Guidelines (PDF, 426 KB),  you have a responsibility to review the disclosure form in a timely manner and discuss it with the employee.

Next steps

Attach or describe:

  • A copy of the employee's disclosure
  • The letter or email (written communication) provided to the employee regarding the outcome or resolution to this matter
  • Any other formal communication provided to the employee (e.g. letter of expectation, emails, etc.)

Submission instructions

Once the form is complete use the Submit button at the bottom. A copy will be sent to you, the employee and the employees personnel file at the BCPSA
